To register as a new patient

To register with the Practice you will need to live in the Practice catchment area. You will need to complete a GMS1 form (available from reception) and produce photo ID and a utility bill from the last 3 months.

After you have registered you will need to attend a new patient check. If you are on medication your new patient check will need to be with a doctor, if not it will be with a nurse.

Priority will be given to those who are new to the catchment area. If you are just changing surgery there will be an 8 week wait.

Our practice catchment area is:

Orsett, Horndon, Bulphan, West Tilbury, Linford, Chadwell St. Mary, Stanford, North Stifford, Grays (with some exclusions.)
