To register as a new patient

If you wish to register with the practice and live within our practice catchment area, please complete the NHS and surgery registration forms (available from reception).

Alternatively, you can register using the new online form – LINK. Once your registration has been accepted, you will need to come into the surgery with proof of id.

After you have registered you will need to attend a new patient check. If you are on medication, your new patient check will need to be with a doctor, if not, it will be with a nurse.

Priority will be given to those who are new to the catchment area.

Our practice catchment area is:

Orsett, Horndon-on-the-Hill (with some exclusions), Bulphan (with some exclusions), West Tilbury, Linford, Chadwell St. Mary, Stanford-le-Hope (with some exclusions), North Stifford (with some exclusions), Grays (with some exclusions.)
